The Centers for Medicare and Medicaid Services (CMS) has recently announced a new revised ABN form CMS-R-131.  Providers may implement ths form immediately but implementation by January 1, 2012 is mandated by CMS.  Incorrect use of the ABN form will result in Medicare classifying the ABN as invalid.  This includes not only using an old form, but also failing to have the patient check one of the boxes describing the services rendered and payment options.
If you use preprinted Physicians Laboratory forms for Paps please be sure to discontinue using the ABN form that is printed on the back.  Click on the link below to print an updated ABN.   Also available is a booklet with more information about ABNs.

If you are a provider-based clinic you are responsible for printing and using your own ABNs.
